The Health Access Networks (HAN)

THE THREE COMPONENTS OF THE PROGRAM

1. Information Sharing/Dissemination

  • To share resource information about health care programs and access-related issues
  • To share information about changes in programs, practices and policies from DMA, DPH and other state or federal health-care related agencies in order to provide accurate and appropriate information to consumers
  • To keep regional providers and outreach staff informed of developments regarding health care programs, access, outreach and services

2. Development of Best Practices

  • To share outreach practices that work to engage and inform people in need of health care and for enrolling people in health care programs. Best practices to be taken from other programs and initiatives within the region, Massachusetts and across the nation

3. Serve As Link Between Communities, State Agencies and Institutions

  • To provide state agencies such as DMA, DPH or other health care-related institutions/ organizations with links to local providers
  • To provide a forum for community-based groups and state-funded providers to directly and productively communicate with state agencies and institutions
  • To provide a forum for problem-solving challenges or problems regarding eligibility, coverage and service utilization encountered by field-based providers
  • To provide a mechanism to assist state agencies and institutions develop and/or implement campaigns to increase enrollment and access to health coverage programs
  • To identify gaps or problems related to eligibility, coverage and service utilization and to provide clear and timely feedback to the appropriate entities regarding solutions which can promote community-based outreach and access

WHO PARTICIPATES IN THE PROGRAM_

  • Community groups engaged in local outreach efforts
  • Representatives from local providers including Community Health Centers, Physicians offices, Managed Care organizations
  • Hospital staff engaged in enrollment and outreach issues
  • State programs concerned with Public Health, Medicaid enrollment, WIC, etc.
  • Representatives from citizen initiatives providing local health solutions/services
  • School nurses
  • All other parties interested in issues of health care access and outreach
